Enrollment Application form

This form is created in Microsoft Word and has been partially protected so that parts of the document can be written on, while other parts cannot. This form can be filled in, saved, and printed for you to bring to enrollment at your appropriate building.

Tips to completing the form:

  • Save the Enrollment Application Word document to your computer before you start to fill it in.
     
  • Type your comments in the grey text boxes by clicking on the box. When it turns black, enter text just as in any Word document.
     
  • You can use the tab key to advance to the next field in the form.
     
  • You will need to complete a form for each school that your children attend. For example, if you have 2 children attending Washington Elementary, one attending Cottonwood, and another attending West, you will need to print one form for the two in Washington, one for Cottonwood, and another for West.
