FACILITY REQUEST

USD 480 welcomes all groups/organizations/people to share in the facilities we have.

School facilities are available for rent to public agencies and to responsible and properly organized community groups for the purpose of education, elections, and civic welfare. All requests must be made to the requested facility administrator 14 days prior to the scheduled event/activity and are subject to the guidelines outlined in the  Facility Request and Contract Agreement. All lessees must provide a Certificate of Liability Insurance naming USD 480 as a certificate holder on the $1,000,000 policy. Rental rates and categories for rates are attached as well as a copy of the Use of Facility Request Procedures.

NOTE: School-related functions and events take precedence over rental requests from outside organizations.

Below are the requesting forms, contracts, guidelines, and policies we have in place for such requests.  Summer requests should read the District Energy Policy.  Days facilities are available are limited during the summer due to this policy.  You may take the completed forms to the building you are wishing to request usage of, or return to the Auxiliary Services office at USD 480 Central Office, 401 N. Kansas.   Or email completed form to Angeles Ruiz.

IN DISTRICT WORK ORDERS

USD 480 utilizes the School Dude work order system to better serve the staff and students of USD 480.  Staff may enter work orders from any computer from the http://www.myschoolbuilding.com/ website.  Login is Required.

 

DISTRICT DEPARTMENT FORMS, POLICIES, and PROCEDURES

Department support material for all School Dude resources and other department forms, policies, and procedures can be accessed through the district's SHAREPOINT document system.  Login is Required.
